Specialised support at home
Patients with GPs in the Harrogate district can be referred to our community team, including people living in residential and nursing homes. Our community team includes palliative care clinical nurse specialists, social workers, occupational therapists and doctors.
This service is advisory, aiming to work in partnership with the patient’s GP, who retains responsibility for patient care. The aims of the service are to ensure symptoms are well controlled and psycho/social needs met. Our team liaises closely with other community services to maximise patients’ wellbeing. On occasions team members may refer to our other services.
If appropriate our staff will also help a patient understand their health and care options so they can plan for the future, including what they would like to happen when they enter the terminal phase.
Members of the team visit patients and their families in their homes making recommendations and providing support that addresses their physical, social, emotional, psychological and spiritual needs. Each case is individual but we often visit or telephone at regular intervals to offer ongoing support to a person through the course of their illness.
Patients, their carers and professionals can also contact our team for advice both in and out of office hours.
